StakeOS asks: What work is happening, who has authority, what does the evidence suggest, what decision was made, what event followed, and what outcome should return to the intelligence layer?

Keeping those responsibilities distinct prevents the operating interface from becoming the source of truth and prevents AI recommendations from becoming unaccountable decisions.
